A major new clinical trial is underway that could help doctors determine which lung cancer patients will truly benefit from immunotherapy, potentially saving thousands from ineffective treatments and harsh side effects. The Phase 2 study, launched by a leading cancer research center, will enroll 535 patients with advanced non-small cell lung cancer, the most common form of the disease accounting for 87 percent of cases.
The six-year IMMUNO-BIOMAP trial is backed by a $23.7 million federal award from the Advanced Research Projects Agency for Health. It aims to map how tumors change as they begin resisting immune checkpoint inhibitors, drugs that target the PD-L1 protein on cancer cells. Currently, fewer than 40 percent of patients see positive results from these therapies, and many eventually develop resistance. Researchers will use sophisticated predictive models and study biomarkers, biological clues in the blood, to tailor treatments to each patient’s evolving tumor biology.
In the second phase of the trial, doctors will roll out new therapies guided by these biomarkers in real time. The goal is to halt disease progression and boost success rates, especially for patients whose tumors have specific genetic mutations that drive cancer growth. The study will also explore whether combining immunotherapy with other treatments works better than chemotherapy alone. “Measuring tumor DNA levels in the blood over time will help us pinpoint which patients face a greater risk for drug resistance and relapse,” said principal investigator Jyoti Malhotra, M.D., associate professor at the research center.
Hope for More Personalized Lung Cancer Care
For patients with advanced non-small cell lung cancer, the trial could mark a shift from one-size-fits-all treatment to a more dynamic, personalized approach. Researchers will focus on how second-line immunotherapy drugs tackle resistance, particularly in patients whose cancers shed malignant cells into the bloodstream before visible changes appear on scans. This phenomenon underscores the need for more precise and timely monitoring. “Our ultimate goal is to identify groups of patients who respond well to personalized treatments and biomarker-guided therapies, allowing them to live longer without relapse,” said lead principal investigator Ravi Salgia, M.D., Ph.D.
The study draws on a network of nearly 40 clinical sites across the United States, ensuring a diverse cross-section of patients. With more than 200,000 people diagnosed with lung cancer each year in the U.S., researchers are optimistic that the insights gained will transform how doctors manage care. “Our findings offer hope for greater treatment success and longer lives,” Dr. Salgia added. The trial is part of a broader $142 million initiative to integrate the latest technologies with tumor biology expertise, paving the way for cancer care that evolves with the patient’s disease.
